Pastoral

This reference form should be completed by a pastor, elder, or deacon at your church who has known you for at least one year and is not a member of your immediate family. Fill out the box at the top of the reference form before giving it to your reference. As a courtesy, please provide the individual with an addressed envelope and postage. Reenrolling students must submit a pastoral reference if two or more semesters at Calvary have been missed.

Personal

This reference form should be completed by a teacher, co-worker, school official, employer, ministry director, etc. who has known you for the past two years and is not a member of your immediate family. Fill out the box at the top of the reference form before giving it to your reference. As a courtesy, please provide the individual with an addressed envelope and postage. Reenrolling students are not required to submit a personal reference.
